What is the Ockenden report

The long awaited Ockenden report has been published this week. This is a landmark independent review that has exposed catastrophic, systemic failures in NHS maternity services. Initially investigating the Shrewsbury and Telford Hospital NHS Trust, and extended via further crucial findings, the inquiry sadly revealed that hundreds of mothers and babies suffered avoidable harm, including stillbirths, neonatal deaths, and severe brain injuries.

The data uncovered by the inquiry is very distressing. Across more than a decade of care, investigators identified 162 potentially avoidable deaths. Furthermore, the review found that 105 infants suffered severe brain damage after being starved of oxygen at birth, while others developed permanent neurological conditions like cerebral palsy due to substandard clinical management.

According to the Ockenden Review Findings, these tragedies were rarely isolated incidents.
