Inquest into the death of Nadeem Khan
Listed 20th April 2010 for 3 weeks at Preston.

The Inquest into the Death of Nadeem Khan (Dean Khan) is due to commence on Tuesday the 3rd February 2009 at Lancaster Hall before HM Coroner for Preston and West Lancashire.

Dean died on the 30th June 2007 having been arrested and taken into custody at Burnley Police Station.

The Police had been called by several residents in the Brierfield area following concerns raised about Dean's bizarre behaviour.

When the Police arrived on the scene Dean was arrested, handcuffed and restrained.  An ambulance arrived at the scene but Dean was not examined.  He was then transferred to Burnley Police Station where he was restrained in a cell, whereupon he collapsed and died.

The Family hope that the Inquest will answer concerns surrounding:

  • The force used by the Police in arresting and detaining Dean
  • The period of time he was restrained for
  • The apparent failure to recognise the condition known as excited delirium and to deal with Mr Khan as a medical emergency.


Inquest into the death of Paul Davies
Listed 12th October 2009 for 1 week at Bridgend.


The Inquest into the death of Mr Paul Davies is listed to take place on the 12th October 2009 for an estimated length of 5 days.

Mr Davies died on the 28th September 2006 at his home on the 26th September 2006 at approximately 7.15am following a search carried out by South Wales Police.

This search was carried out following intelligence and the issue of a search warrant.  The police operation was to target a number of properties in the same area that day and the matter was classed as a 'low level drugs warrant' with no major risks attached to it.

Whilst the Police were searching the property, Mr Davies swallowed a package, unfortunately the package became lodged in his throat before he fell unconscious. Mr Davies never regained consciousness and was pronounced dead at 12.25am on the 28th September 2006.
 
In addition it has become apparent that in November 2005 a similar search was carried out and on that occasion Mr Davies also swallowed an unknown package which resulted in him having similar difficulties breathing.  On that occasion Officers began immediate first aid and the package became dislodged.

However following this incident it appears that the police failed to update their intelligence log and PNC details of Paul Davies and failed to note that he had a risk of swallowing packages.

The family will hope that the Inquest will answer concerns regarding:

  • The failure to update intelligence logs
  • Consideration of the risks involved prior to searches
  • The first aid provided to Mr Davies


Inquest into the death of Sean Coffey
Listed 7th October 2009 for 3 weeks at Liverpool.

The Inquest into the death of Mr Sean Michael Coffey commences on Monday 12th March 2009, before HM Coroner for Liverpool, Andre Rebello. The Inquest is sitting in the Liverpool Coroner's Court and is listed for a period of three weeks.

In September 2006, Mr Coffey was transferred to HMP Liverpool following difficulties at HMP Forest Bank. Whilst in HMP Liverpool Mr Coffey suffered from withdrawal symptoms and depression.

He threatened to self-harm and commit suicide on a number of occasions, both verbally to Prison Staff and in letters. Mr Coffeys medical records were not transferred from HMP Forest Bank to HMP Liverpool, the staff could therefore not appreciate the severity of Mr Coffeys ill health.

Mr Coffey was offered a detoxification programme at HMP Forest Bank however refused, as his previous experience of the programme was unsuccessful.

At 11.40 on the 29th November 2006 Mr Coffey was found hanging in his cell, resuscitation attempts were made but unsuccessful.   

The family hopes the Inquest will examine amongst other things:

  • The reason as to why Mr Coffeys medical records were not transferred with him.
  • The medical treatment Mr Coffey was given
  • Suitability of the drug withdrawal programme
  • Failure to put Mr Coffey on a self harm watch at HMP Manchester despite him indicating he wanted to self harm

Inquest into the death of Richard Baldwin
Listed 30th November 2009 for 5 days at Peterborough.


Mr Baldwin was arrested on Friday the 9th December 2005 and appeared in the Magistrates Court the next day, thereafter remanded into HMP Peterborough due to reappear at Court on Monday the 12th December.

Mr Baldwin was being treated for a leg injury and it was suspected he carried MRSA. Mr Baldwin was extremely ill and in the morning he was found semi-conscious and an ambulance was called.  He died at hospital. 

The family hopes the Inquest will examine:

  • The healthcare treatment that Mr Baldwin received
  • Why there wasnt medical intervention at an earlier stage
  • The lack of staff on duty that evening


Inquest into the death of Emmanuel Buyoya
Listed 8th February 2010 for 6 weeks at Bridgend.

Mr Buyoya was 20 years old when he died. Mr Buyoya appeared at Cardiff's Magistrate's Court on 23 December 2005 where he was charged remanded to HMP and YOI Parc in Bridgend.

It was his first time in prison and he had no previous criminal convictions.

He was discovered in his cell on the segregation unit on the 29th June 2006 hanging, suspended by a bed sheet, later that day he was sadly pronounced dead.

The family wants the Inquest to examine:

  • Mr Buyoyas treatment in prison
  • The bullying and racism that Mr Buyoya was suffering
  • What changes the prison have implemented since Mr Buyoyas death in order to prevent future deaths and ill treatment
